Community Engagement

We partner with local, national community-based organizations (CBOs), not-for profit organizations (NPOs), faith-based organizations, and non-government organizations (NGOs), to engage community members through the trusted relationships they have established locally.

Rapid Deployment

We bring healthcare services to underserved communities where and how it is needed, by identifying providers and public health entities to deploy in hard to reach/engage communities.

Technology & Innovation

We connect providers, CBOs, and public health on a common platform to collect consistent data across sites of care, share information, and establish a sustainable method to demonstrate impact.

Strategy for Success

Local communities are at the center of the Health Equity Consortium’s success strategy. Health Equity Consortium participant organizations provide technical assistance and help with securing funding opportunities, providing technology tools, introducing local partners, and developing operational roadmaps to allow communities to design and build strategies that address their unique needs and circumstances.

COVID Catchup

The Health Equity Consortium designs programs to reach underserved populations that missed essential health services during COVID and may qualify for reimbursement from federal COVID programs.

Community Health Hubs

The Health Equity Consortium can help you design and staff community health hubs and health-hub popups, including call center outreach, staffing, data collection and data sharing.

The pandemic has exposed long-standing inequalities in healthcare and created a stark contrast between the haves and have-nots.
